Sky High Party Rentals is pleased to partner with Principle Health Systems to administer COVID-19 Pfizer vaccinations at our Oak Forest location. To ensure you receive a vaccination please register for an appointment as soon as possible as there are limited slots.

The registration process is two (2) STEPS

First, click on the Schedule an Appointment link below to sign up for a vaccination appointment.

STEP 1 Schedule an Appointment Slot

Second, click on the Patient Registration link below to complete the patient intake form. Registrants are required to complete the registration form PRIOR to their appointment.

STEP 2 Patient Registration 

A registration form must be completed for each patient, i.e., a family of four will need to make four appointments and complete four patient registration forms. Sign ups must be completed no later than 9PM the day prior to the appointment.
