Sky High Party Rentals is pleased to partner with Principle Health Systems to administer COVID-19 Pfizer vaccinations at our Oak Forest location. To ensure you receive a vaccination please register for an appointment as soon as possible as there are limited slots.

The registration process is two (2) STEPS

First, click on the Schedule an Appointment link below to sign up for a vaccination appointment.

STEP 1 Schedule an Appointment Slot

Second, click on the Patient Registration link below to complete the patient intake form. Registrants are required to complete the registration form PRIOR to their appointment.

STEP 2 Patient Registration

A registration form must be completed for each patient, i.e., a family of four will need to make four appointments and complete four patient registration forms. Sign ups must be completed no later than 9PM the day prior to the appointment.

We were very pleased with the whole process from ordering it to delivery and pickup. We ordered the huge Star Wars obstacle course with the water slides at the end. The kids had so much fun at the whole party. Alfiado was very helpful in measuring our yard to make sure it would fit, and he was there during delivery and pickup. Highly recommend!
